Caffeine and PMU: What the Research Actually Says

Should you skip your coffee before PMU?
The honest answer is more nuanced than the usual “no caffeine” rule. Caffeine is not alcohol, and the scientific evidence does not support treating a standard cup of coffee as an automatic reason to cancel an appointment. Still, caffeine can affect blood pressure, anxiety, tremor, alertness, and a client’s ability to sit comfortably and still. For some clients, especially those who are caffeine-sensitive or prone to anxiety, a strong coffee or energy drink right before PMU can make the appointment harder than it needs to be.
The best recommendation is usually moderation, timing, and individualized pre-care. Good artists explain the why, not just the rule.
What Caffeine
Actually Does
Caffeine is a stimulant that primarily works by blocking adenosine receptors in the nervous system. Adenosine is involved in sleep pressure, relaxation, and pain modulation. When caffeine blocks those receptors, people may feel more alert and less tired. Depending on dose and individual sensitivity, they may also experience increased heart rate, palpitations, restlessness, shakiness, anxiety, or difficulty sitting still.
The effect is not identical for everyone. Genetics, body size, habitual caffeine intake, sleep quality, medications, anxiety history, pregnancy status, and the amount and timing of caffeine all influence the response.
Research also suggests that regular caffeine users can develop tolerance to some of caffeine’s effects. In one study, caffeine increased alertness in habitual consumers, but this benefit could be offset by increased anxiety and jitteriness.
That distinction matters in PMU. The goal is not to make clients sleepy or uncomfortable. The goal is to help them arrive calm, hydrated, able to communicate clearly, and able to remain still throughout a detailed procedure.
Does Caffeine Increase Bleeding?
This is where social-media advice often becomes too absolute.
The claim that caffeine “thins the blood” is not well supported by the available research. Caffeine is not an anticoagulant, and it should not be presented as equivalent to alcohol, aspirin, prescription blood thinners, or antiplatelet medications.
A study involving 13 healthy young adults who consumed 750 mg of caffeine per day for seven days found no statistically significant changes in platelet aggregation. The researchers did observe a temporary increase in diastolic blood pressure after the initial caffeine intake, but that effect disappeared with continued consumption.
That is important because it means a routine coffee is not clearly proven to increase procedural bleeding through impaired platelet function.
Some newer studies have found that acute caffeine can alter clotting-related responses in the specific setting of strenuous exercise, particularly among caffeine-naive individuals. However, these are exercise-hemostasis studies, not PMU studies, and they do not demonstrate that a cup of coffee before a cosmetic tattoo causes increased bleeding or bruising.
Energy drinks deserve separate consideration. They are not simply coffee in a can. In a small crossover study of 32 healthy adults, consuming a 16-ounce energy drink increased arachidonic-acid induced platelet aggregation compared with water after 60 minutes. Energy drinks often contain high caffeine doses plus other stimulants and ingredients. They may increase jitters, palpitations, anxiety, and dehydration risk for some clients.
The evidence-based message is this: caffeine is not clearly a bleeding-risk issue in the way alcohol or certain medications can be. But very high caffeine intake, energy drinks, and individual sensitivity can make the client experience and treatment conditions less predictable.
Pain Sensitivity Is Not Simple
Another common claim is that caffeine automatically makes PMU hurt more. The science does not support such a blanket statement.
Caffeine’s relationship with pain is complex. It can act differently depending on the dose, whether the person is a regular caffeine user, whether they are in withdrawal, their sleep quality, and the type of pain being studied.
In a small randomized, double-blind study, one cup of coffee containing approximately 100 mg of caffeine did not significantly change electrical, mechanical, or cold pain thresholds compared with decaffeinated coffee in healthy participants.
That is a useful reality check. A single ordinary coffee is not proven to make every client more pain-sensitive during PMU.
On the other hand, caffeine can increase anxiety and physiological arousal in susceptible people, which may make any discomfort feel more intense. A review of caffeine and mental health found that high doses can induce anxiety, and that people with panic disorder or performance social anxiety may be particularly sensitive to caffeine’s anxiogenic effects.
There is also a practical issue with sudden caffeine restriction. Regular coffee drinkers who skip caffeine completely may experience withdrawal symptoms such as headache, fatigue, irritability, and reduced alertness. Research on habitual caffeine consumers has documented withdrawal-related sleepiness, lower alertness, and poorer performance on some cognitive tasks.
So a blanket “no caffeine at all” policy can backfire. A client who normally drinks coffee every morning may arrive with a headache, low energy, irritability, or withdrawal symptoms. That does not create a more comfortable appointment.
Stillness Matters More Than Most Clients Realize
PMU requires precision. Artists work on highly visible areas, often close to the eyes, lips, or delicate brow tissue. Even small movements can affect symmetry, line quality, saturation, and the artist’s ability to work efficiently.
Caffeine does not make every person shaky. But caffeine-related restlessness, trembling, fidgeting, anxiety, or a racing heartbeat can make it harder for some clients to stay relaxed in the chair.
Evidence from energy-drink research illustrates the issue. A review reported that adults commonly described jitteriness, restlessness, or shaking hands after energy-drink consumption. In the studies analyzed, energy-drink use was associated with greater odds of insomnia and jitteriness or increased activity compared with control beverages.
That does not mean coffee and energy drinks should be treated as identical. It does mean that the client who arrives after a large cold brew, a double espresso, and an energy drink may have a very different appointment experience from someone who had a modest morning coffee with breakfast.
For an artist, this is less about policing caffeine and more about supporting conditions that promote calm, comfort, and control.
A Moderate Recommendation Makes Sense
For most clients, the evidence supports moderation rather than complete elimination.
A practical PMU pre-care recommendation may look like this:
- Avoid high-dose caffeine and energy drinks on the morning of your appointment.
- If you are caffeine-sensitive, anxious, prone to palpitations, or tend to shake after coffee, skip it or choose decaf.
- If you are a regular coffee drinker, consider reducing your usual intake rather than suddenly stopping it and arriving with withdrawal symptoms.
- Eat a balanced meal and drink water before your appointment.
- Avoid combining caffeine with alcohol, recreational drugs, or non-prescribed stimulants.
- Tell your artist about relevant medications, medical conditions, anxiety, panic symptoms, fainting history, or previous reactions to PMU procedures.
Artists should also avoid giving medical instructions that exceed their scope. Clients should not stop prescribed medication, including anticoagulants, stimulant medication, or anxiety medication, without consulting the clinician who prescribed it.
Caffeine and Alcohol Are Different
Alcohol guidance before PMU is more clear-cut because alcohol can impair platelet-related clotting processes and disrupt aspects of early wound repair. Caffeine does not have the same evidence profile.
The most accurate comparison is this:

The nuance matters. Clients deserve recommendations based on realistic risk, not fear-based rules.
Make It Part of the Consultation
A good consultation gives clients the reason behind the instruction.
Instead of saying, “No caffeine,” consider saying:
“You do not necessarily need to skip your normal morning coffee. But please avoid large amounts of caffeine or energy drinks before your appointment. We want you hydrated, comfortable, and able to stay relaxed and still. If coffee makes you shaky, anxious, or prone to palpitations, choose decaf or skip it that morning.”
That explanation is more likely to earn compliance because it is honest. It also leaves room for individual needs rather than treating every client the same.

Coffee lovers, this one is for you: caffeine is not automatically off-limits before PMU.
Current research does not show that a standard cup of coffee universally increases bleeding, bruising, pain, or pigment loss.
